ABSTRACT
The study focuses on describing and analyzing the existing system for birth and death registration within a healthcare institution, which currently operates manually. The system relies on two primary data collection and storage mechanisms: the 'Book of Life' and the 'Theatre Book.' The 'Book of Life' records data during birth registration and updates it with death information post-mortem, while the 'Theatre Book' contains details related to births, including hospital name, member information, date, age, and sex. The manual nature of the system creates inefficiencies in data handling and storage, emphasizing the need for evaluation and improvement.
To comprehensively understand the existing system, fact-finding techniques were employed. These included interviews with staff, patients, and doctors to gather firsthand information on processes such as newborn registration, daily birth registration volume, storage methods, and document issuance. Observational methods provided insights into real-time operations, enabling the identification of procedural gaps. Additionally, extensive research was conducted to develop meaningful recommendations and ensure the validity of findings. The study highlights the limitations of the current manual processes and sets the foundation for designing an improved system.
